Accidental Death and Dismemberment Insurance*

You will be covered for the Accidental Death Insurance that you selected on your Membership Enrollment/Agreement Form.

This group insurance product is not available in Kansas or Maine.

You can rest easy knowing you've helped secure your family. Coverage is in effect 24 hours a day, 7 days a week, 365 days a year - whether at home, on business or outside of the country.

Submitting a Claim Is Easy

To file a claim, please contact the Plan Administrator at this toll-free number: 1-855-595-6489 (Monday - Friday, 7am - 8pm CT).

Reduction of Benefit Amount - If an Insured Person is age 70 or older on the date of an Accident causing Loss, then the Benefit Amount payable will reduce according to the following schedule:

Payment Card Protection*

Protect all your credit, debit, ATM and check cards in the event of loss or theft. You'll feel more secure knowing that you can get help to cancel your cards, and stop unauthorized purchases, 24 hours a day.

IMPORTANT EMERGENCY FEATURES

If your cards are lost or stolen and you're more than 5O miles away from home, you may also request these services when you call to report your missing cards:

Emergency Cash Advance - Obtain up to $300 in a cash advance to help you out in a pinch. Pick up your cash at one of over 245,000 Western Union locations in 200 countries.
Emergency Airline Tickets - If theft or loss leaves you without cash or credit cards, you can receive an airline ticket to get home.

Important Note: You will be invoiced within 30 days of your request for the amount of the cash advance and/or cost of the airline ticket.

IMPORTANT NOTICE: YOUR LIABILITY FOR FRAUDULENT CHARGES UNDER FEDERAL LAW:

For credit cards: If the card issuer has notified you of your maximum potential liability has provided a means for you to notify the card issuer of credit card loss or theft and if the credit card contains a way of identifying the cardholder or authorized user, then your liability of unauthorized use or your card before the card issuer is notified is no more than $50.00 on each card.

For debit and cash-machine (ATM) cards: Your liability for unauthorized use of your card is no more than $50.00 if you notify the card issuer of card loss within two business days after you learn of the loss of the card. After that your liability is up to $500.00 provided that the card issuer establishes that the unauthorized charges would not have occurred if you had notified the card issuer within the two business day period. ln addition, if you do not notify the card issuer within 60 days after a periodic statement showing unauthorized transfers is sent to you, then you will also be liable for the amount of unauthorized transfers that occurred after the 60-day period and before notice to the card issuer, provided that the card issuer establishes that the unauthorized charges would not have occurred if you had notified the card issuer within the 60-day period. Your card issuer's liability policy may provide for lesser liability amounts than indicated above. Consult your card issuers terms and conditions for specific details. The policy of many card Issuers is not to hold card holders liable. Nothing set forth in your membership materials alters any rights you may have under federal or state law with respect to unauthorized or erroneous transactions on your card accounts. You are not required to have this service to maintain your credit, debit, or ATM cards.

Key Ring Protection*

Help to retrieve lost keys. When you request your member key ring, it will be registered so lost keys can be returned by mail, postage paid!

Protect your keys without compromising security or privacy. Keep all your keys on the specially registered key ring. If your keys are lost and returned to the Member Service Center, they'll promptly send them to you, postage paid.

Fill out the Payment Card Protection Registration Form, located inside the New Account Kit. Mail the completed form in the envelope provided, or enclose in a stamped envelope and mail to the address provided on the form.

Key Ring Protection:

Check the Key Ring box on the Benefits Request Form included with the New Account Kit and mail in the enclosed envelope.

Key Ring Protection provides you with a personally registered key ring, so your lost keys can be dropped in a mailbox and returned to you at no additional charge.
